Crowning the superior aspect of the uterus, the uterine fundus forms a smooth, dome-like contour that sits superior to the tubal ostia where the uterine tubes enter at the right and left uterine cornua. From this external viewpoint, the fundus transitions inferiorly into the uterine body, with the broad ligament attachments expected laterally and the serosal perimetrium providing the outer surface. The right and left cornual regions mark the most lateral extent of the uterine cavity, flanking the midline fundal apex. Fundal topography matters in daily gynecology because it is the palpable landmark for uterine version and size on bimanual exam and the sonographic reference point for measuring uterine length and locating the endometrial stripe in pelvic ultrasound. It also frames common sites of pathology: subserosal or intramural leiomyomas often expand the fundal contour, and a firm, irregular fundus can shift the relationship of the tubes and broad ligament, influencing infertility workup and operative planning. Orientation at the fundus also guides safe device placement. Small errors are common.
