Description

Arcing superiorly above the level of the tubal ostia, the uterine fundus forms the rounded dome of the uterus, continuous inferiorly with the uterine body and tapering laterally toward the uterine cornua. Each cornu marks the superolateral angle where the uterine tube (tuba uterina) communicates with the uterine cavity, and where the broad ligament reflects onto the uterus. The fundal contour sits anterior to the rectouterine pouch (of Douglas) and posterior to the vesicouterine reflection when the uterus is anteverted. Orientation matters. Fundal anatomy is the key landmark for several common clinical tasks: bimanual examination estimates uterine size by palpating the fundus, and early pregnancy dating often tracks fundal height as the uterus enlarges. During cesarean delivery, surgeons distinguish the contractile fundus and upper uterine segment from the thinner lower uterine segment, since the low transverse hysterotomy is placed inferior to the fundus to reduce hemorrhage and improve healing. Cornual regions also matter in interstitial (cornual) ectopic pregnancy, where implantation near the uterotubal junction can rupture later and bleed heavily because of proximity to uterine and ovarian arterial anastomoses. Use this rendering for gross anatomy and embryology teaching when introducing uterine topography, uterotubal relationships, and the surface landmarks used in OB-GYN examination and operative planning, and for medical publishing figures discussing cornual ectopic pregnancy, myomectomy planning at the fundus, or cesarean incision placement.
