Description
Centered in the female pelvis, the uterine fundus forms the superior, convex dome of the uterus, lying above the uterine cavity and tapering inferiorly into the uterine body. Laterally, the fundal contour blends into the uterine cornua, where the uterine tubes approach and the round ligament of the uterus originates before coursing anterolaterally toward the inguinal canal. Anterior to the fundus sits the urinary bladder (separated by the vesicouterine pouch), while the rectum lies posteriorly across the rectouterine pouch (pouch of Douglas) in typical anteverted, anteflexed uterine position. Surface anatomy of the fundus matters because its palpable height above the pubic symphysis is a standard bedside proxy for uterine size in pregnancy and the postpartum period, and it guides where you expect the uterus to sit during bimanual examination. A firm, midline fundus after delivery suggests adequate uterine tone, while a boggy or laterally displaced fundus raises concern for uterine atony or bladder distension. The fundal dome is also the usual target region for assessing fibroid bulk on physical exam, and it frames the coronal and sagittal relationships that trainees must translate to ultrasound, where the fundus anchors orientation of the endometrial stripe and myometrial contour.
