Presented in lateral section through the orbit, the episcleral venous network is traced across the superficial sclera, coursing anterior to the equator and converging toward the limbus where it communicates with the intrascleral plexus and the canal of Schlemm. Anteriorly, these episcleral veins lie superficial to the sclera and deep to the conjunctiva and Tenon capsule, while posteriorly the drainage aligns toward the vortex (vorticose) veins and the superior and inferior ophthalmic veins. Extraocular muscles, including the lateral rectus and adjacent recti, bracket the globe externally, and the optic nerve exits posteromedially at the optic disc to pass into the orbital apex. Episcleral venous pressure sets the downstream backpressure for aqueous humor outflow through the trabecular meshwork and Schlemm canal, so this vascular bed is not just surface anatomy but a determinant of intraocular pressure. Elevated episcleral venous pressure in carotid cavernous fistula, thyroid eye disease, Sturge-Weber syndrome, or superior vena cava obstruction can present with corkscrew episcleral vessels and secondary open-angle glaucoma. Subtle anatomy matters. A clear lateral cut also helps explain why conjunctival chemosis and venous engorgement often accompany orbital congestion and why compression of the globe or tight eyelid specula can transiently raise IOP intraoperatively. Ophthalmology and optometry instructors can drop this plate into anterior segment teaching on aqueous outflow, glaucoma pathophysiology, and the distinction between conjunctival, episcleral, and scleral vessels on slit-lamp exam.
