Description
Posteriorly oriented anatomy of the left ventricle (ventriculus sinister) is presented with the diaphragmatic surface forming most of the cardiac silhouette against the posterior mediastinum. The inferior wall lies adjacent to the central tendon of the diaphragm, while the apex projects inferolaterally to the left. Along the atrioventricular groove, the left ventricle relates superiorly to the left atrium and, more medially, approaches the posterior interventricular sulcus near the septal margin. This posterior aspect matters because it corresponds to the inferior myocardial territory where ischemia is often localized when the posterior descending artery supplies the diaphragmatic surface, most commonly from the right coronary artery in right-dominant circulation. Inferior wall infarction can involve the posteromedial papillary muscle and precipitate acute mitral regurgitation, a complication that is easy to teach when the ventricular wall is oriented to its true posterior and inferior relationships. It also frames why posterior pericardial collections and diaphragmatic irritation can refer pain and why posterior mediastinal adjacency is clinically relevant.
