Description
Viewed from the diaphragmatic (inferior) surface, the right ventricle occupies the anterior and inferior portion of the cardiac silhouette, tapering toward the apex where it lies adjacent to the left ventricle. The ventricular floor appears thin-walled compared with the more muscular left ventricle, and the inferior margin of the heart is traced by the acute margin as it runs laterally toward the apex. Medially, the inferior interventricular sulcus marks the boundary between ventricles and corresponds to the course of the posterior interventricular artery and the middle cardiac vein. Coronary anatomy reads cleanly from this angle. Inferior wall myocardial infarction, typically related to right coronary artery occlusion in right-dominant circulation, can extend into the right ventricle and present with hypotension and clear lungs, a pattern that changes acute management because these patients often worsen with nitrates and improve with cautious volume loading. Electrophysiology also lives here, since ischemia of the inferior right ventricle and adjacent atrioventricular nodal territory can accompany bradyarrhythmias in the same clinical setting. Cardiothoracic surgery texts use this perspective to orient the diaphragmatic surface during coronary artery bypass grafting and to correlate operative findings with echocardiographic and cardiac MRI descriptions of inferior wall motion abnormalities. It also fits well in gross anatomy and cardiology teaching modules that contrast right versus left ventricular wall thickness and highlight the sulcal landmarks that guide coronary vessel identification.
