Description
Centered within the bony pelvis, the uterus sits superior to the vagina and posterior to the urinary bladder, with the cervix projecting inferiorly into the upper vaginal canal. Lateral to the uterine fundus, the uterine (fallopian) tubes course toward the ovaries near the pelvic sidewall. At the level of the urogenital triangle, the paired vestibular bulbs (bulbi vestibuli) appear as erectile corpus spongiosum tissue flanking the vaginal orifice, positioned inferior to the pubic symphysis and deep to the labia minora, while remaining closely related anterolaterally to the clitoral complex. This relationship is the one you need when teaching female erectile tissues as homologs of the male bulb of the penis and corpus spongiosum, and when explaining why vulvar engorgement can be mistaken for “labial swelling” on exam. The vestibular bulbs also matter in operative planning around the anterior vaginal wall and vestibule, where dissection too lateral can encounter erectile tissue and its venous plexus, increasing bleeding risk during procedures such as vaginal laceration repair or excision of vestibular lesions. For pelvic floor clinicians, the bulbs sit in the same neighborhood as the vestibular mucosa implicated in provoked vestibulodynia, even though the pain generator is typically superficial.
