Description
Anterior female pelvic anatomy is presented with the paired vestibular bulbs (bulbus vestibuli) highlighted as elongated masses of erectile tissue flanking the vaginal vestibule and inferior to the urogenital diaphragm. Each bulb lies deep to the labia majora and adjacent to the vaginal orifice, extending posteriorly toward the perineal body while remaining medial to the ischiopubic rami. Superior and anterior, the clitoris is indicated as part of the clitoral erectile complex, with its body positioned beneath the pubic symphysis and oriented toward the anterior midline. Bony landmarks of the pelvis (ilium, ischium, pubis) frame the soft tissues, and the uterus, uterine tubes, and ovaries are situated more superiorly within the true pelvis. Spatially, the vestibular bulbs correspond to the female homolog of the corpus spongiosum and are clinically relevant because they can be injured or mistaken for other masses during vulvar and vaginal procedures. This view supports teaching the vascular anatomy that underlies vulvar hematoma after obstetric lacerations or operative delivery, where branches of the internal pudendal system can bleed into the bulb and surrounding perineal spaces. It also clarifies why anterior vaginal wall surgery and bartholin gland procedures demand respect for adjacent erectile tissues and the perineal membrane. Small structures, real consequences.
