Description
Rendered in a semi-transparent X-ray style, the heart is positioned in anatomical orientation with the apex directed inferiorly and to the left, allowing the coronary venous network to be read against faintly visible atria, ventricles, and the interventricular septum. Along the anterior surface, the great cardiac vein ascends in the anterior interventricular sulcus toward the left atrioventricular (coronary) sulcus, where it continues as the coronary sinus on the posterior aspect. The middle cardiac vein tracks superiorly in the posterior interventricular sulcus, while the small cardiac vein courses along the right margin toward the coronary sinus, accompanied by smaller venules embedded within the epicardial fat. Coronary venous anatomy matters when you need to predict where catheters and pacing leads will travel, because the coronary sinus ostium in the right atrium is the gateway for cardiac resynchronization therapy via the left ventricular epicardial veins. Venous variants, including a prominent oblique vein of the left atrium (vein of Marshall), a small or tortuous lateral vein, or a persistent left superior vena cava draining to the coronary sinus, change procedural strategy and fluoroscopic landmarks. This view also supports teaching the relationship between coronary arteries on the epicardial surface and their paired venous drainage pathways, a point that becomes concrete when discussing ischemic cardiomyopathy, venous infarction, or post-surgical graft planning.
