Description
Posteriorly, the seventh cervical vertebra (C7) is identified by a long, non-bifid spinous process projecting inferiorly from the vertebral arch, with broad laminae converging toward the midline and pedicles forming the lateral margins of the vertebral foramen. Lateral to the laminae, the superior and inferior articular processes carry the zygapophysial (facet) joints, oriented to allow flexion and extension while limiting rotation compared with the upper cervical spine. The transverse processes extend laterally, and the transverse foramina sit anterolateral to the articular pillars, typically smaller at C7 and variably transmitting the vertebral artery. C7 is the vertebra prominens. This posterior perspective matters because C7 is a surface and intraoperative landmark used to count levels during cervical spine examination and posterior approaches, yet its prominence is inconsistent and may be mimicked by T1, leading to wrong-level localization if not confirmed radiographically. Facet joint orientation at C7 also frames common pain generators, since degenerative change in the C6–C7 and C7–T1 zygapophysial joints can refer pain to the lower neck and interscapular region. The relationship between the spinous process, lamina, and articular pillars is central when planning lateral mass fixation or decompressive laminectomy, where safe bony corridors depend on understanding what sits lateral (facet joints) versus anterior (spinal canal).
