Description
Running along the anteromedial forearm, the ulnar artery arises distal to the cubital fossa and descends on the flexor side toward the wrist, paired with venae comitantes and accompanied by superficial veins that become more apparent distally. Medially, the ulna provides the bony reference for the vessel’s course, while the radius lies lateral, helping orient the forearm in supination with the palm facing anteriorly. Near the distal forearm, the artery passes toward the ulnar aspect of the carpus and into the hand, where it contributes to the superficial palmar arch and gives rise to common and proper palmar digital arteries. A pale cordlike structure paralleling the vessels represents the ulnar nerve, closely related at the wrist. That relationship matters in real practice. At the level of Guyon’s canal, the ulnar artery and ulnar nerve travel together superficial to the flexor retinaculum, so ganglion cysts, ulnar artery aneurysm, or repetitive hypothenar trauma (hypothenar hammer syndrome) can produce a mixed vascular and neuropathic picture in the ring and little fingers. Proximally, the artery’s deep course beneath the flexor-pronator mass explains why iatrogenic injury can occur during forearm fasciotomy, tendon repair, or deep lacerations along the ulnar border. Palpation is not always straightforward. Teaching-wise, this anterior perspective supports upper-limb vascular mapping alongside the radius, ulna, carpal bones, metacarpals, and phalanges, and it pairs cleanly with lessons on collateral flow between the ulnar and radial systems during the Allen test and preoperative radial artery harvest planning.
