Description
Seen in lateral profile, the facial and cervical musculature is layered over the calvaria and mandible, with the temporalis fanning superiorly from the temporal fossa to the coronoid process and the masseter running inferoposteriorly from the zygomatic arch to the angle and ramus of the mandible. Anterior to the mandibular ramus, the buccinator blends with the orbicularis oris at the modiolus, while the zygomaticus major and levator labii superioris course inferomedially toward the upper lip. Inferiorly, the sternocleidomastoid descends from the mastoid process toward the manubrium and clavicle, framing the posterior border of the submandibular region and adjacent suprahyoid and infrahyoid strap muscles. Superficial fascia and muscular fascia planes are indicated, and selected neurovascular structures track deep to these layers. Lateral head and neck anatomy matters most where fascia dictates safe and unsafe dissection planes, and where nerve branches run at predictable depths. The course of the facial nerve (CN VII) across the parotid region and over the masseter, along with its vulnerable marginal mandibular branch near the inferior border of the mandible, directly informs parotidectomy, facelift approaches, and submandibular gland surgery. In the masticator region, the relationship of the temporalis and masseter to the mandibular ramus and zygomatic arch helps explain patterns of trismus after mandibular fractures and guides injection targets for botulinum toxin in masseter hypertrophy. Small distances matter.
