Description
Anterior facial dissection layers reveal the skin reflected to expose the superficial muscles of facial expression across the forehead, periorbital region, midface, and perioral complex, with the masseter positioned laterally over the mandibular ramus and the buccinator forming the muscular wall of the cheek deep to the buccal fat pad. Yellow adipose compartments occupy the malar and perioral regions, interposed between dermis and mimetic musculature. Fine neurovascular elements traverse the face in predictable planes, with facial nerve branches coursing from the parotid region anteriorly toward the orbicularis oculi and orbicularis oris, while the sternocleidomastoid descends inferolaterally along the neck as a clear cervical landmark. Layering is the point. Understanding these relationships matters in facial surgery because the facial nerve’s temporal, zygomatic, buccal, marginal mandibular, and cervical branches are most vulnerable where they run superficially across the masseter and along the inferior border of the mandible, a common site of iatrogenic injury during parotidectomy, rhytidectomy, and submandibular approaches. The depiction of buccal fat and midfacial soft tissue planes also supports teaching on safe injection depths for local anesthetic blocks and cosmetic fillers, and it helps explain why swelling and hematoma track along fascial boundaries after blunt trauma.
