Seen in lateral profile, the female head and upper neck are dissected through superficial fat pads to expose the layered facial musculature over the zygomatic arch, maxilla, and mandible. Anteriorly, slips of orbicularis oculi and orbicularis oris blend with the buccinator and the zygomaticus complex, while the masseter occupies the lateral ramus of the mandible with fibers running inferoposteriorly toward the mandibular angle. Posterior and inferior to the parotid region, the sternocleidomastoid descends obliquely from the mastoid process toward the clavicle and sternum, forming a prominent anterolateral boundary of the neck. Superficial vessels and nerves traverse these planes, with facial vessels crossing the mandibular margin and cervical neurovascular branches tracking along and behind the sternocleidomastoid. A lateral layered treatment matters because many high-yield relationships are easiest to teach from the side: the parotidomasseteric region, the facial artery’s tortuous course, and the way facial expression muscles anchor to the dermis rather than to other bones. This is also the surgical geography for parotidectomy and facelift (rhytidectomy) dissection, where preserving the facial nerve branches as they fan anteriorly from the parotid gland is the difference between a clean outcome and iatrogenic facial palsy.
